Propósito: Describir el rol de las enfermeras en las unidades de hospitalización domiciliaria (HH) en los hospitales públicos de Chile. Métodos: Se realizó un estudio descriptivo exploratorio en 26 enfermeras de unidades de HH de 14 hospitales públicos de Chile. Los datos se recogieron mediante una encuesta telefónica y/o por correo electrónico. Resultados: El rol relacionado con la gestión/administración de los cuidados de enfermería fue el más comúnmente desempeñado por las enfermeras que trabajan en HH (92,3%). Las enfermeras de los HH también desempeñan un papel en el ámbito educativo y de los cuidados clínicos. Ninguna de las enfermeras desempeñó un papel en la investigación.Conclusiones: Es necesaria la promoción de la formación continuada de las enfermeras que trabajan en HH, ya que es la única forma de aumentar su producción científica y conseguir un mayor reconocimiento profesional y proporcionar a los usuarios de HH una mejor atención.

Autoimmune limbic encephalitis is defined as a severe inflammatory disorder in the brain that generates a rapidly progressive encephalopathy usually in less than 6 weeks. Antibodies against epitopes of synaptic surface proteins or against intracellular proteins have been described. Six cases of autoimmune encephalitis are reported with a description of each one including typical manifestations, imaging changes, clinical course, complications during hospitalization and response to treatment. Two of the cases were confirmed by antibody isolation. One died during hospitalization. Our experience shows a high risk of sequelae and refractoriness to treatment depending on the time of initiation of therapy.

Extensive pseudomembrane adhered to the colon wall in a patient with C. difficile infection. Radiological tests provide unspecific data that could correspond to any type of colitis, so although they do not allow us to diagnose the disease, they can guide us in the appropriate clinical context. We will consider them in case we want to rule out other intra-abdominal pathologies or if we suspect any complication. In simple radiology we will be able to observe a thickening of the haustras with signs of fingerprints suggestive of submucosal edema or distension of the colon. It is also useful to rule out complications such as ileus, perforation, the presence of megacolon or even acute pseudo-obstruction74. In the CT scan we will also identify a thickened colon wall, and after contrast injection, the «target sign», in relation to concentric rings with different attenuation values due to mucosal hyperemia and submucosal edema53,74. 53,74 Occasionally a detached pseudomembrane or nodular thickening may be observed. Another finding when oral contrast is administered is the «accordion sign», in which the contrast is trapped between thickened haustras75. In more severe cases we will observe ascites. Patchy distribution and especially involvement of the rectosigmoid may point to a diagnosis of C. difficile infection.
